The minimum required monthly payment is 5% of the outstanding balance.
How to Request a Payment Plan
To request a payment plan:
- Complete the Payment Plan Application - Download.
- Email completed application to [email protected].
If approved, the payment plan agreement will be issued electronically within approximately 3 business days.
Important Information
- Applications missing required information will not be processed.
- Submission of a payment plan application does not guarantee approval.
- State tax refunds may continue to be offset while a balance remains due, even if a payment plan is approved.
- Failure to comply with an approved payment plan may result in enforcement action, as allowed by law.
